By accessing dental.evergrowleads.com ("the Site") and submitting an appointment request, you agree to be bound by these Terms of Service ("Terms"). These Terms are entered into between you and Evergrow Leads LLC ("we," "us," or "our"), which operates this Site on behalf of Advanced Dental Care of Fraser, MI ("the Practice"). If you do not agree to these Terms, please do not use this Site.
2. Purpose of This Site
This Site exists solely to facilitate new patient appointment requests for Advanced Dental Care located at 15275 E 13 Mile Rd, Fraser, MI 48026. By submitting the form on this Site, you are requesting that a representative of the Practice contact you to schedule an appointment — you are not booking a confirmed appointment until the Practice confirms your time slot directly.
3. The $79 New Patient Special
The $79 New Patient Special (comprehensive exam, digital X-rays, and professional cleaning) is subject to the following conditions:
- Available to new patients only — patients who have not previously been treated at Advanced Dental Care
- Valid in the absence of periodontal disease — if periodontal disease is diagnosed during the exam, additional treatment may be recommended and priced separately
- This offer expires September 1, 2026 and may be discontinued at any time
- Cannot be combined with insurance benefits for the same services
- The Practice reserves the right to determine eligibility at the time of the appointment
4. SMS Communications and Consent
By submitting your phone number through the form on this Site, you expressly consent to receive SMS text messages from Evergrow Leads LLC and Advanced Dental Care of Fraser at the number provided. These messages may include:
- Transactional messages — appointment request confirmations, booking confirmations, and scheduling updates
- Informational/reminder messages — appointment reminders sent 24 hours and 2 hours before your scheduled appointment
- Follow-up messages — follow-up communications related to your appointment request if you have not yet scheduled
Message frequency varies. Message and data rates may apply. You may opt out at any time by replying STOP to any message. Reply HELP for assistance. Consent to receive SMS messages is not a condition of receiving dental services.
5. Accuracy of Information
You agree to provide accurate, complete, and current information when submitting the appointment request form. Providing false or misleading information may result in your appointment request being declined.
6. No Medical Advice
Nothing on this Site constitutes medical or dental advice. All clinical decisions, diagnoses, and treatment recommendations will be made exclusively by licensed dental professionals at Advanced Dental Care. The information on this Site is for appointment scheduling purposes only.
7. Limitation of Liability
To the fullest extent permitted by applicable law, Evergrow Leads LLC shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of this Site or the appointment booking process. Our total liability to you for any claim arising from your use of this Site shall not exceed $100.
8. Third-Party Services
This Site uses third-party services including GoHighLevel for CRM and appointment management, and Netlify for website hosting. Your use of this Site is also subject to the terms of these third-party providers. We are not responsible for the practices of these third-party services.
9. Age Restriction
You must be at least 18 years of age to submit an appointment request through this Site and consent to receive SMS messages. By submitting the form, you confirm that you are 18 years of age or older. Parents or legal guardians may submit on behalf of minors.
10. Carrier Liability Disclaimer
Evergrow Leads LLC and Advanced Dental Care are not liable for any delays or failures in the receipt of SMS messages. Delivery of SMS messages is subject to the terms and conditions of your wireless carrier. Message and data rates may apply. Neither we nor the wireless carriers shall be liable for any delays in the receipt of, or failure to deliver, any SMS message.
11. Intellectual Property
All content on this Site, including text, graphics, and design, is the property of Evergrow Leads LLC or Advanced Dental Care and is protected by applicable intellectual property laws. You may not reproduce or distribute any content from this Site without prior written permission.
12. Governing Law
